How We Are Transforming Healthcare Interoperability with AI

September 25, 2025

When most people think of Generative AI in healthcare, their minds jump to AI-powered chatbots or large language models (LLMs) summarizing clinical notes. While those applications hold future promise, at Medicom, we are focused on a more immediate and profound revolution that’s happening behind the scenes. We are putting AI to work in the foundational “digital plumbing” of health systems to solve a challenge that has plagued healthcare for decades: enterprise imaging interoperability.

During a presentation at the AWS booth at HIMSS 2025, our CEO, Michael Rosenberg, pulled back the curtain on this new frontier. Our focus isn’t on building another clinician-facing tool, but on leveraging Generative AI to fundamentally rewire how hospitals share and access crucial imaging data.

From Manual Chaos to Automated Clarity

For anyone who has worked in a hospital’s scheduling or records department, the process of retrieving a patient’s prior medical images is a familiar pain point. Before an upcoming oncology consult or a pre-operative appointment, staff embark on a manual, labor-intensive hunt involving phone calls, faxes, and slow data imports.

The historical solution to this relied on massive, handcrafted mapping files to connect different procedure codes. But as our CEO explained, “For anybody that’s ever worked with a compendium of radiology procedures or orders, you know, it’s a huge challenge. It’s a huge pain.” These mappings were brittle, breaking every time a hospital’s compendium changed, and required months of work to build and maintain.

That’s precisely what we’ve done with our Smart Search solution: replaced that entire convoluted process with an LLM we trained on over 50 million radiology procedures. Instead of a rigid map, Smart Search uses a sophisticated AI that understands the language and context of radiology, automatically finding relevant prior exams for a patient’s upcoming appointment.

Power of an AI-Driven Workflow

This shift from manual mappings to an intelligent, learning model delivers transformative benefits for the health systems we partner with:

  • Integration in Days, Not Months: The struggle of spending “months and months” arguing about compendiums is over. With Smart Search, a health system can simply connect its HL7 interface, and our LLM works “right out of the box” because it already understands the nuances of radiology procedures.

  • Adaptability by Design: When a hospital frequently updates its procedure compendium, the old mapping systems would break. Our LLM, however, adapts seamlessly. It understands the meaning behind the procedures, not just a rigid code, making it resilient to change.

  • Superior Accuracy and Context: Time and time again, clinical reviews have shown that our LLM outperforms handcrafted mappings. It finds relevant priors that old systems missed and provides a deeper contextual understanding, ensuring clinicians have a more complete picture of their patient’s history.

The AWS Backbone: How We Built It

Building on a robust, cloud-native AWS environment provides the essential foundation that makes our entire workflow possible. We leverage a suite of powerful services on AWS to train our AI, standardize the data using the modern FHIR format, and deliver the results directly into the EHR. This seamless, end-to-end process ensures clinicians get the right data at the right time, without leaving their native workflow.

AI in Action

This isn’t just theory; it’s in practice today. A Florida-based acquisitive health system uses Medicom Smart Search to bridge the data gap between its main hospital and the numerous outpatient radiology practices it has acquired. Instead of a costly data migration, our system automatically looks across all their facilities to present relevant priors directly in their EHR, driving a workflow that was previously manual and fragmented.

Our ultimate goal is simple but profound: meet users where they are. By using the power of Generative AI and the scalability of AWS, we can automate these incredibly complex background workflows, eliminating the need for clinicians to bounce between applications and ensuring they have the right information, right inside the patient’s chart, every single time.
